I think " A brother/sister is like a shoulder means your brother and sister are there for you no matter what. It doesn't necessary mean there your blood sisters or brothers. They can be a real good friend who's like a sister/brother. They're there for you to lean on. So when you describe someone as a shoulder your saying that they're always there for you.
